There are US-made CNC produced rear trunnions available from James Dunst, [Only registered and activated users can see links. ] for $45 plus $3.85 priority mail shipping.
I have seen one, and it looks perfect, pretty much a copy of the late generation Soviet investment cast part.

I bought one on Gunbroker to replace the "trimmed" rear block that came with my ORF "special edition gas piston" PMKMS kit. The machine work is nice and it's milled from a solid block of steel. The holes line up exactly with the Polish piece, so fit and operation will be terrific. I give this item a BIG thumbs up!!!
