The 2009 undercover operation "Fast and Furious" by the Bureau of Alcohol, Tobacco, Firearms and Explosives was so badly botched that both Congress and the Obama administration must take steps to ensure such a tragedy never again occurs. Thus far, no one seems prepared to seriously address the problem.
